Background of the Study
The concept of a therapeutic milieu refers to a structured and supportive environment within psychiatric settings that fosters recovery and promotes positive behavioral changes (Smith et al., 2024). It integrates interpersonal relationships, structured activities, and a safe physical environment to enhance patients' emotional, social, and psychological well-being (Henderson & Johnson, 2023).
At Jos University Teaching Hospital, Plateau State, psychiatric wards are designed to provide care for individuals with various mental health conditions. However, the extent to which the therapeutic milieu influences patient recovery remains underexplored. A supportive environment can significantly affect the recovery trajectory, enabling patients to regain functionality and improve coping mechanisms.
Given the rising prevalence of mental health disorders in Nigeria, there is an urgent need to optimize psychiatric care environments. This study examines the impact of therapeutic milieus on patient recovery, providing insights into effective strategies for enhancing psychiatric care.
Statement of the Problem
Mental health disorders pose significant challenges to public health systems, and recovery often requires a holistic approach that extends beyond medical treatments. In psychiatric wards, the therapeutic milieu is a crucial yet often overlooked component of patient recovery.
Despite the importance of this concept, limited attention has been paid to evaluating its effectiveness in the Nigerian context. At Jos University Teaching Hospital, the absence of empirical data on the role of therapeutic milieus in psychiatric wards hinders improvements in care quality. This study aims to address this gap by exploring the relationship between therapeutic milieus and recovery outcomes.
Objectives of the Study
1. To assess the components of the therapeutic milieu in psychiatric wards at Jos University Teaching Hospital.
2. To evaluate the influence of the therapeutic milieu on patients’ recovery in psychiatric wards.
3. To identify barriers to creating an effective therapeutic milieu in psychiatric wards.

Scope and Limitations of the Study
This study will focus on psychiatric patients admitted to Jos University Teaching Hospital’s wards from 2023 to 2025. Data will be collected through structured observations, interviews with healthcare professionals, and patient recovery assessments. Limitations include variability in patient conditions and potential biases in self-reported data.
